SB.CCM createAnswer: Maximum number of T.38 calls or PLC/VAD already running

The TA is rejecting T.38 requests due to "SB.CCM createAnswer: Maximum number of T.38 calls or PLC/VAD already running".  There are no other fax or modem calls active.  What would cause this?
