How do I request a Wireless Regulatory Compliance for a BSC-600 with firmware 6.5.1.03?

I have two clients with BSC-600 controllers that I would like to upgrade to 6.5.1.03.  I have loaded the firmware but before I can cut it over I need the Wireless Regulatory Compliance code (Canada).  How do I go about requesting the codes and what information is required?

Unfortunately the BSC is no longer a supported platform, and we unable to provide country codes at this time.
