Per our customer the switch stack has rebooted three times in the past month. The customer does operate in the GUI but advised that they were not logged in when the problem occurs. I notice that the switch shows POWER-ON-RESET after the reboot. Any thoughts?

I too am interested in the 'System returned to ROM' status. I'm familiar with Warm Start when issuing the reload command, and Hard Reset after loss of power. Not sure about Power-On Reset.
It is generally recommended to run the latest software release from the Extended Maintenance Branch; R11.4.3 as of this post. I believe several important fixes have been provided and it's always a good place to start.
