Adtran 908 SIP Options Ping

I am getting a SIP Options Request ping from our Metaswitch SBC which will determine if the trunk is "active" and ready to place calls.

Now I cannot seem to get this to happen. I have followed advice in this thread: SIP OPTIONS request gets answered by a 501 Not Implemented and then I can see the options pings are responding OK

Metaswitch still says I am down:

Check your IP routing, also that media-gateway ip is set properly on the interface.

It looks like you may be traversing a NAT with a public 209.81.107.134 on on side and private 10.4.49.202 on the other.

Can you IP ping with a source of 10.4.49.202 and a destination of 209.81.107.134 ?
